If I forget to take it completely throws me of balance . As for Concerta it doesn't get covered by our medical insurance in the Netherlands. So for me it's a financial choice as well. We find the extended has the spikes and at times doesn't feel like it's working at all. I take the regular every 3.5 hrs. so I don't feel the "coming off"" bit. As for the crude bit: people who take the regular release form tend to experience spikes in the drug's effectiveness shortly after they take it and the drug's effects don't last as long. But I've heard a lot of other ADHDers who don't respond to it well. I also constatnly forget to take the second dose, so the prescriptions that I get now are for extended release. If they see good changes then you know it is working and if they see bad ones you know you need to talk to your doctor.
